Abstract
This study aims to analyze the causes of failure in the DC-AC inverter used in the back up power system at the Cibatu substation. The inverter under investigation is a module equipped with protection systems against overload, overvoltage, short circuit, and overtemperature. Despite being equipped with various protection systems, there was a fire incident indicating a failure in the system. The method used to identify the root causes of the problem is Root Cause Analysis (RCA). RCA was conducted through several stages, including visual inspection, operational and environmental data collection, analysis of the protection system, and interviews with technicians and maintenance staff. The data collected were analyzed to identify factors contributing to the inverter failure. The research results show that low material quality is the main cause of DC-AC inverter failure. Apart from that, the old age of the components, especially considering that the Cibatu Main Substation has been operating since 1996, and physical damage to the inverter module, also contributed to the fire incident. Recommendations resulting from this research include replacing components with higher quality materials, improving maintenance procedures, and implementing stricter monitoring of operational conditions to prevent similar failures in the future.
